ConnectorProvider and Form

Hi,

Currently i devlop an Custom Connector with “ConnectorProvider” and parameters inerited from AbstractConnectorRequest implementation, that’s ok.
But now my customer ask me to add an Form if parameters are missing in my Connector !
Do you have an example to join Connector with Form ?
